Capturing Online Patient Medical Information
Some IVF clinics wish to capture online patient medical information. This
page presents some considerations and possible alternatives.
Considerations
If you wish to capture online patient medical information, here are some
important considerations:
- What's the advantage of collecting the digital information online? For
example, will you migrate that information into your internal patient medical
record system (if you have one)? If so, how will you do that? You would need
to map fields from one system to another system.
- Will you be following the strict HIPPA requirements for
security and privacy? You may want to review these pages published by the U.S.
Department of Health & Human Services:
- For patients that need to answer the questions in their computer in
two sittings, how will they maintain their information
online if they have to leave the web form. If you provide a password to them,
so they can come back to their partially completed forms, then you'll need to
find a way to maintain their information over time, on the server.
- Is all of the above worth the "bang for the buck?" Are there other
alternatives?
Alternatives to Capturing the Information Online
If the above considerations seem rather large, IHR offers the following
alternatives to capturing patient medical information online:
- Convert your MS Word files (e.g. 10-page Patient Medical Form) into a PDF
files. You can then upload the PDF files to your website for patients to
download to their computer, print the form, handwrite their answers on the
form, and return the completed form to your office. These two IHR pages

- Here is a fancier method. You can convert your MS Word file into a PDF
form file which works as an online form, as follows: You create a PDF form
with data fields which a user can actually file in and save to their computer.
The advantage here is that the user types their information into the form,
which looks better than handwritten information.
